This package includes an evaluation form (Part 1) and a selection justification form (Part 2).  Both forms come with instructions and a suggested procedure on performing the evaluation and selection process.  

The evaluation form provides a structure that can be altered to your own evaluation criteria and correlates to the justification form where all subcontractors' earned points are documented exposing the subcontractor with the highest point value.  The final executed forms provide a record of the evaluation and the final subcontractor selected.
